On May 29, 2024, Thompson Coburn became aware of suspicious activity within our network. We promptly launched an investigation with the assistance of third-party forensic specialists. The investigation determined that certain files stored within our environment were viewed or taken by an unauthorized actor between May 28, 2024, and May 29, 2024. A detailed review of the affected files was undertaken and through that review, we determined that certain protected health information related to certain patients of PHS was contained within those files.
While Thompson Coburn has no indication of identity theft or fraud in relation to this incident, the review determined that the following types of information were present in the affected files: name, Social Security number, date of birth, medical record number, patient account number, prescription/treatment information, clinical information, medical provider information, and health insurance information. The specific type of information at issue varies for each individual, and is specified in the letters that were mailed to the involved individuals.
Thompson Coburn is notifying potentially involved individuals through this website posting, issuing a media press release where required in order to comply with the law, and mailing letters to involved individuals for whom we have a valid address. Out of an abundance of caution, the involved individuals were offered access to credit monitoring and identity theft protection services, free of charge.
Upon becoming aware of this incident, Thompson Coburn promptly took steps to investigate the incident and implemented additional security enhancements to further protect against similar incidents.
While there is no indication of identity theft or fraud in relation to this incident, as a precautionary measure, potentially involved individuals are encouraged to remain vigilant against incidents of identity theft by reviewing account statements, credit reports, and explanations of benefits for unusual activity and to detect errors.
